Annia Aurelia Galeria Lucilla was the second daughter to Marcus Aurelius and Faustina the Younger. Her younger brother was Commodus. In 164 AD, she married Marcus' adoptive brother and co-emperor Lucius Veras, which is alluded to in the film by the fact that her son, Lucius, is named for his father. She was widowed when Lucius Veras died in 169 AD. Unlike in Gladiator, Annia Lucilla did not remain a widow, but remarried to the older Tiberius Claudius Pompeianus Quintianus of Antioch.

Unlike in the film where Lucilla joins the plot for the good of the Roman people, she participated in a plot because she lusted after power. In 182 AD, she was involved in a plot with her cousin, the former consul Marcus Ummidius Quadratus, to assassinate Commodus and raise her husband up as Emperor. However, the assassin was not to be a gladiator or general, but Pompeianus' nephew. The attempt was thwarted and the conspiracy unraveled. Commodus exiled Annia Lucilla to the island of Capri, but soon ordered her executed, so unlike the film she did not outlive her brother.
